DEPARTMENT SUMMARY: The Los Angeles LGBT Center’s Department of Health Services is one of the nation’s largest and most experienced providers of LGBTQ-specialized primary care and related services, Our mission to help LGBTQ people lead healthier, happier lives is supported by our comprehensive array of client-centered services including primary care, mental health, psychiatry, sexual health, pharmacy, case management, nutrition, clinical research, housing services and a food pantry. The Center is one of a handful of Federally Qualified Health Centers in the nation specializing in serving LGBTQ people.
JOB SUMMARY: The Nurse Practitioner IIII (NPIII) will have a demonstrated history of excellence in clinical practice. In this role, the NPIII will independently provide comprehensive, trauma-informed, and culturally competent medical care to LGBT individuals throughout the adult lifespan. The NPIIIs panel of patients will reflect the diversity of LGBT community and include individuals of all genders and expressions and with varying levels of medical and psychosocial complexity, including those living with HIV. The NPIII will provide episodic care for non-panel patients as needed. In this role, the NPIII will continue to have regular support from an MD/DO if requested and the opportunity to meet with their clinical supervisor. In this role, the NPIII will also contribute to training and mentorship of others in the practice.
ESSENTIAL FUNCTIONS:
• Obtain review of psychotropic medications and mental health history, provide a 5-axis differential diagnosis, and formulate and execute a treatment plan including medications and follow-up schedule, including need for referral to specific specialists.
• Contribute to patient chart maintenance by regularly reviewing and updating patient problem lists, medications, and health maintenance section of the EHR.
• Complete and sign-off on clinical notes, lab results, refill requests,and imaging reports, and consult within a timely manner and in accordance with practice standards.
• Respond to messages from patients and care team members in a timely manner.
• Provide evidence-based care that is in accordance with national standards and internal practice guidelines.
• Seek consultation from staff psychiatrist as needed, especially when a patient’s problems are especially complex or if the patient’s condition is not responding as expected.
• Participate in peer review exercises and complete mandatory clinical trainings.
• Liaise with other members of the clinical care team including nursing, social work, and mental health to optimize patient health outcomes.
• Participate in the afterhours call pool once four psychiatric providers become eligible.
• Provide cross-coverage for the other psychiatric providers in cases of vacations, conferences, and illness, and overflow, including
review of other providers’ lab results, portal messages, and medication refill requests.
• Participate in training, education, and mentorship activities in the clinic including but not limited to new provider onboarding, student/trainee teaching, staff meetings, and general provider education.
• Maintain sufficient Continuing Education Units (CEU) as required by the California Board of Nursing for license renewals and as may be required to maintain any certifications necessary to demonstrate proficiency in a specialty area relevant to NPs clinical practice.
• Other duties as assigned.
JOB QUALIFICATIONS AND EXPERIENCE:
• Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.
• A passion for the Center’s work and its mission to make the world a better place for LGBTQ people.
• CA RN license/MSN with current CA Furnishing Number.
• ANCC/ California Nurse Practitioner Certification.
• Licensed to practice as a 103NP Nurse Practitioner by the California Board of Nursing.
• At least 3700 hours working as a nurse practitioner providing psychiatric care
• Completion of a performance review and clinical audit that demonstrates competence and evidence-based practice in clinical assessment, decision making and documentation.
• Active DEA license and up-to-date BLS certification.
• Minimum 5 years’ experience with outpatient mental health care or HIV specialty mental health care with dual-diagnosis.
• Bilingual skills a plus (English/Spanish fluency preferred).
• Ability to develop and maintain patient records.
• Proficiency with word processing, database operations, and spreadsheets.
• Ability to effectively educate patients and their families
• Superior interpersonal communication skills and writing skills.
• Ability to work independently with minimal supervision.
• Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
